Description:

We are seeking a dedicated Transition Specialist to oversee transition and reentry practices within juvenile correctional facility operations. In this role, you will work closely with staff, external agencies, and students to guide individualized education and community reentry planning, supporting successful reintegration into public schools, employment, or training programs.

Key Responsibilities
• Curriculum & Practice Standardization:
o Facilitate the implementation of life skills curricula (e.g., Next Steps).
o Utilize standardized coordinator handbooks to maintain consistent transition practices across facilities.
• Reentry Planning & Case Management:
o Evaluate transition consultations completed by students under assigned instructional guidance.
o Participate in weekly administrative release/reentry committees to confirm and reinforce Reentry Plans.
o Assist in developing Individualized Education/Learning Plans upon youth entry.
• Agency Collaboration & Community Services:
o Partner with state, county, and private organizations to deliver community reentry services.
o Support collaborative initiatives with workforce development, mentoring, and employment programs.
• Student Advocacy & Support:
o Assist students in acquiring essential identification and legal documentation (e.g., social security cards, state IDs, driver’s licenses, birth certificates).
o Facilitate transitions back into public school systems, vocational/apprenticeship programs, employment, or military pathways.
• Follow-up & Representation:
o Conduct post-release follow-up calls with former students at 1, 3, 6, 9, and 12-month intervals.
o Represent the facility and youth services department at relevant meetings, conferences, and community forums.

Qualifications & Attributes
• Strong background in youth mentorship, education administration, juvenile justice, or social services.
• Excellent communication skills for interfacing with youth, multi-agency partners, and committee boards.
• Strong organizational skills and ability to manage follow-up schedules and student documentation.

Required / Desired Skills

  • Prior working exp. utilizing strong communication skills (written and verbal). (Required 1 Years)
  • Computer knowledge: Microsoft Word, Outlook, and Excel as well as PowerPoint. (Required 1 Years)
  • Prior experience working as a customer service rep. (Required 1 Years)
  • Prior data entry working experience. (Required 1 Years)
  • Prior experience working in a Team environment. (Required 1 Years)
  • Prior exp. working w/at risk youth. (Highly Desired 1 Years)
  • Prior working experience in a community or State program. (Highly Desired 1 Years)
  • Prior experience working in a youth educational program. (Highly Desired 1 Years)
